Surgical Resection of PAVMs (Pulmonary Arterio-Venous Malformations) with High Risk of Embolotherapy - Case Report -

Abstract
Pulmonary arteriovenous malformations (PAVMs) are thin walled aneurysm caused by abnormal communication between the pulmonary arteries and veins. They may occur primarily or in association with hereditary hemorrhagic telangiectasia (HHT). They act as direct right-to-left shunts, resulting in dyspnea, fatigue, cyanosis, and polycythemia when the shunt is large. Although embolotherapy, using detachable balloons or stainless steel coils, is generally accepted as the best choice for patients with multiple PAVMs, treatment for the solitary PAVMs remains a subject of debate. We present 43-year-old woman with a large solitary PAVM centrally located and in high risk of percutaneous transcatheter embolization. She underwent successful surgical resection (right middle lobectomy) without complications.
